The Des Plaines Park District needs your input to understand your use and opinions of park district programs, facilities, and parks. By completing this survey, you will provide key input to help shape the future strategic plan for parks and recreation within the Des Plaines Park District.

• The survey should take less than 10 minutes to complete.
• Your feedback is confidential.
• A summary of the results will be available to residents to view once the project is completed

Everyone who completes the survey will have the option to submit their email address for one of twenty
